Does anyone know what this is? I tried to read how it should works but I genuinely didn't understand.

from what i read, it's a piece of elastic band with holes (the wider bottom piece), connected to a thinner non-elastic shoelace-like string, with a bead threaded through to allow the string to tighten around the penile shaft.

it looks like you would put the shaft into the loop formed with the string, and bring it to the base of the shaft. the elastic band would be tied around the scrotum, or pass between the legs below the scrotum and looped around one thigh, securing the band to a static part of the body. the ends of the string part should now be located at the base of the shaft, where you would retract the foreskin all the way to the base of the shaft, and tighten the string-loop using the bead to prevent the foreskin from rolling forward.

at least, that's my vague interpretation of how it's used..
