You reach "permanently retracted" status when your foreskin doesn't roll forward by itself and doesn't need a support to hold it behind glans even during normal sexual activities. I am permanently retracted for 25+ years (I do not feel any movement of my foreskin). Over many years, my foreskin has become a ring-of-skin at the back of corona. My foreskin rolls back automatically if I apply pressure to pull it forward. I believe it is the old Japanese definition of "permanently retracted man". If you want to achieve permanent retraction then, based on my training, keep your glans exposed at all time (especially during sleep at night when body tend to adjust more). Wearing a tight-fit cotton brief underwear may help accelerate the progress, but my trainer warned me that wearing tight brief significantly reduce men's sperm count. It took me more than 2 years to permanently retract. So it takes long time. If you plan to have children later in life then perhaps wearing a tight brief isn't a good idea. In my case, I got tested for my sperm count before marrying my wife, and it was fine.

If your foreskin is rolling forward during sex then your foreskin hasn't shrunk enough to comfortably stay behind your glans, and and your glans hasn't inflated enough to hold your foreskin behind your glans. It takes really very long time to comfortably wear your intact foreskin behind your glans. May take 10+ years of training.

PS. Based on my experience, when you start keeping your glans bare day-and-night continuously everyday (with or without support), you will experience frequent erection and flow of pre-ejaculation fluid during the first several months of your progress. Wearing a tight brief also advantageous at this stage. Change your brief when it gets wet to avoid infections. If you get lots of bruises on your glans and/or inflammation on your foreskin (called edema, put pressure on it to subside the inflammation) then use a lubricant. I used Vaseline.

My trainer used to say that permanent retraction is a cosmetic medical procedure. You need to take care of your glans and foreskin to accept the changed status and smoothly adapt to exposed glans.

I started exposing the glans about 2 years ago, abandoning the venture shortly after. I was on holiday and the sensation of a super sensitive glans rubbing against tissue was too annoying and complicated to manage in that situation. (the skin kept coming back over the glans, so you understand I was constantly having to tidy up)
Now that I have started again (for a month now) with more determination, the glans has quickly lost sensitivity, I often can't tell if it is exposed or covered by the skin when in my underwear. The first two weeks the skin tended to return to its natural position, but now it remains quietly stuck back. Up until now I've only used the cotton pad method to make sure the skin stayed in place overnight (without it I was sure it would) but in general, over a full day it almost never shifts. (it's worth saying that I suffered a slight phimosis, so the skin could be helped to get stuck behind the glans, but I never noticed any swelling)

I can stay retracted by just pulling back now. It's so cool and easy! I think the extra flaring of the head has made it possible, even when masturbating my foreskin doesn't want to go over the head which is perfect.

ive been sleeping nude for a while now and feels great, no issues. every now n then my meatus gets caught on the sheets and have to move it bc its feels sensitive still. tried keeping it there to help desensitize but its hard some times

Tengo 66 años, desde los 14 años llevo el glande completamente descubierto.  Simplemente mi prepucio es corto y a medida que mi pene se fue desarrollando el prepucio fue quedando detrás de la corona del glande. 
Actualmente si cubro el glande con el prepucio, este solo cubre un tercio de la cabeza del pene, pero al soltar los dedos el prepucio retrocede automáticamente y deja la cabeza completamente descubierta.
Por su parte mi prepucio forma un pequeño anillo de piel que queda alojado detrás de la corona de la cabeza.

I am 66 years old, since I was 14 I have had my glans completely uncovered.  My foreskin is simply short and as my penis developed, the foreskin became behind the crown of the glans.
Currently, if I cover the glans with the foreskin, it only covers a third of the head of the penis, but when I release the fingers the foreskin automatically recedes and leaves the head completely uncovered.
For its part, my foreskin forms a small ring of skin that is lodged behind the crown of my head.
The bare headed penis is beautiful.

Well, answering your message, I tell you that in my family we are four brothers, I am the youngest. Both my father and my brothers have the same condition in their penises, they all have the glans exposed. I remember that my paternal grandfather also had his penis with his head uncovered. The first memories that come to mind are the fishing trips that we did as a family, at that time I was about 5 to 6 years old. I remember that I was fascinated, when I saw my father or my brothers urinate, I observed in wonder their penises that to me just a boy, they seemed huge, but what amazed me the most were their big heads, which as I told you I wore naked. At that time I compared it with mine, which seemed so small and with a covered head. My brothers are older than me, they also have their penises with retracted foreskin, which is why I thought that all adult penises should be the same. My father taught me since I was little, that when urinating I had to retract the foreskin, I think that had a great influence on the development of my penis and the glans in particular. As I told on the forum, at the age of 11 I began to practice retraction. At age 14 I was completely withdrawn. What a great story👍In my case, it was my uncle. I saw him naked when I was a teen and was impressed by the beauty of the pulled-back penis. Unfortunately, I didn't dare ask about it and thought it was just a part of his personal anatomy. If I had known that foreskin can be trained, I would have started pulling it back then and would have a shorter foreskin in my adulthood.

My father never advised me to keep the glans uncovered, much less circumcision, even though he has been circumcised since birth (like my grandfather) My parents were well aware of my phimosis (moderate), but the doctor probably convinced them that it was a frequent problem that resolved spontaneously. Now I am firmly convinced that the father (if not in favor of circumcision) must at least teach his son to keep the glans uncovered since puberty. This is an entirely male issue, which children obviously don't understand, but very important for all the reasons we discuss in this forum. If I had started this practice as a child, now my foreskin wouldn't have all this excess skin, and it would be much more functional.

I remember the first days of retraction: I had strong sensations of annoyance, tingling and tickling, which is perhaps a strong discomfort for a child, idk, but it is good to induce and convince him to pursue this path, for his own good as an adult, when the sexual aspect becomes dominant in his life

I couldn't agree more. I'd never circumcise my sons at birth, but I would definitely teach them the pulled back way. It's honestly shocking how little boys are taught about their penis.
